Reasonable Accommodations

If you are an applicant or participant with one or more disabled household members, you may request a reasonable accommodation for any of Public Housing Authority’s rules, policies, practices or services in order to take full advantage of the programs and services that are offered.

How do I request a reasonable accommodation?

If you have a disability you may request a reasonable accommodation by having your physician complete a Reasonable Accommodation Request form and returning it to us. Your doctor or other service provider will need to verify your need for the accommodation.

If you are requesting an accommodation during your eligibility process for housing assistance please return your Reasonable Accommodation Request form to:

City of Tucson – CSD
Housing Assistance
310 N. Commerce Park Loop, PO Box 27210
Tucson, AZ 85726-7210

If you are requesting an accommodation while you are receiving Housing assistance, please instruct your medical provider to return the Reasonable Accommodation Request form to your Housing Services Agent. Please contact us if you need to find out how to reach them.
